Swing Sets Recalled by Kompan Inc. for Repair Due to Fall Hazard
The following product safety recall was voluntarily conducted by the firm in cooperation with the CPSC. Consumers should stop using the product immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.
Name of Product: Swing sets
Units: About 700
Manufacturers: Kompan Inc., of Tacoma, Wash.(from January 1998 through December 2003); and BigToys Inc., of Olympia, Wash.(from January 2004 through December 2008).
Hazard: The joint connection between the horizontal top beam and the vertical end bracket and support post system can crack and break, posing a fall and impact hazard to users.
Incidents/Injuries: Kompan received 19 reports from BigToys of swings that have had the end brackets replaced due to both actual or potential for cracking or breakage at the joint of the top beam and the support posts. The firm has received three reports of minor injuries to users, however the firm has been unable to verify the cause or nature of these injuries.
Description: The recalled swing sets include the “To Fro” models listed below with any numbers in the series of model number codes listed. For example, for the first entry, S1-8, all single bay, 8 foot swings beginning with the model number S1-8 would be included in the recall. Kompan has not manufactured the swing since December 31, 2003 and has not sold it since December 31, 2008.
Sold by: Kompan, Inc. from January 1998 to December 31, 2008 for about $700-$3250. The units were sold to customers through third-party sales representatives throughout the United States. Third-party sales representatives are individuals and companies who market and sell Kompan playground equipment business to business.

Remedy: Consumers should immediately stop using the recalled swing sets and remove the chains connecting the seats to the top beam. A retrofit kit will be provided to all affected customers with installation instructions. Kompan is contacting swing owners directly by telephone and mail to advise them to stop using the swing, remove the chains connecting the seats to the top beam and advising customers that retrofit kits will be delivered with installation instructions. Kompan will follow up by telephone and email after the retrofits are delivered to confirm installation by customers.

No Specific Cause Found Yet Linking Dry Max Diapers to Diaper Rash
WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission (CPSC) and Health Canada (HC) have reviewed consumer incident reports and other information involving Pampers Dry Max diapers. To date, the review has not identified any specific cause linking Dry Max diapers to diaper rash.
Both agencies will continue to evaluate consumer complaints related to Pampers Dry Max diapers and will provide parents with updated information if this assessment changes. As always, parents and caregivers are advised to seek the attention of a medical professional if they have any concerns about adverse health reactions.
From April through August 2010, CPSC received nearly 4,700 incident reports about diaper rash. Nearly 85 percent of these complaints came in May and then dropped off significantly. As part of its technical evaluation, staff from each agency considered certain characteristics of the diaper, including the materials used, the construction of the diaper, and heat and moisture retention issues.
In addition, CPSC staff reviewed clinical and toxicological data found in published, peer-reviewed medical literature. CPSC also critically reviewed data submitted by Procter & Gamble (P&G) and the results of a human cumulative irritation patch study conducted by P&G in May 2010. Further, chemistry, toxicology and pediatric medicine information provided by Health Canada was reviewed by CPSC.
While the investigation thus far does not find a link between the diapers and the health complaints received, CPSC recognizes the serious concerns expressed by parents. CPSC staff cannot rule out that there may exist a health concern for some babies, especially those babies that may be sensitive and develop rashes or other skin problems as a result of contact with the materials in this or other products.
Most babies exhibit diaper rash at least once in their lifetime. If parents or caregivers believe that their child is suffering from a rash that they believe to be related to a diaper, CPSC staff suggests that they discontinue use of the diaper and contact their pediatrician.

Incidents/Injuries: The firm has received a report of an exposed wire in the asparagus. No injuries have been reported.
Description: This recall involves The Land of Nod toy vegetables sets made of felt wool. The toy set has six vegetables: radish, onion, asparagus, tomato, lettuce and carrot. The asparagus is the only toy vegetable involved in this recall.

Midland Farms is voluntarily recalling certain milk products due to the
potential of improper pasteurization. The recalled milk products are all sold
in plastic containers and are marketed under the brand names Midland Farms,

Commissioner Patrick Hooker today alerted consumers that Proper
pasteurization heats milk in order to effectively eliminate all pathogenic
bacteria, such as Listeria and Salmonella.
The recalled milk products are all sold in plastic containers and are marketed

The problem was detected by New York State Department of Agriculture and
Markets' milk inspectors during an inspection on September 7, 2010 when they
found a failure of pasteurization equipment at the Menands milk plant. While
the recalled products have not been found to be contaminated with any harmful
pathogens, the company is voluntarily recalling all production as a
precautionary measure.
No illnesses have been reported to date to this Department in connection with
this problem. Consumers are warned not to use or consume the recalled products
even if it does not appear or smell spoiled.

Abbott recalls infant formula on bug contamination

WASHINGTON – Drugmaker Abbott Laboratories said Wednesday it is recalling millions of containers of its best-selling Similac infant formula that may be contaminated with insect parts.
The voluntary action affects up to 5 million Similac-brand powder formulas sold in the U.S., Puerto Rico, Guam and some Caribbean countries. The company said the products may contain a small beetle or larvae, which could cause stomach ache and digestion problems.
The recall does not affect any liquid formulas or other Abbott-brand products.
A company spokeswoman said Abbott uncovered the insects last week in one section of a manufacturing plant in Sturgis, Michigan.
"We immediately shut down that one area and began an investigation," said Abbott's Melissa Brotz. "We're in the process of resolving it now." Abbott manufactures Similac at several U.S. sites.
Brotz said the company has been consulting with the Food and Drug Administration, which determined there was no "immediate health risk" from the contamination.
Consumers can enter the lot number on their containers online to determine if they are subject to the recall. The products should be returned to Abbott for a full refund.
"Delivering anything less than the highest quality infant formula is unacceptable to us," said Brotz. "We will do whatever is necessary to maintain the trust of parents in the coming weeks."
The affected products were sold in plastic containers and various can sizes, including 8-ounce (227-gram), 12.4-ounce (352-gram) and 12.9-ounce (366-gram).
Similac is the best-selling infant formula in the U.S. Abbott's nutritional products — which include adult brands like Ensure — had sales of $5.3 billion last year.
Abbott expects to lose $100 million in connection with the recall. However, the North Chicago, Illinois, company confirmed its third-quarter earnings guidance between $1.03 and $1.05 per share.
Shares of Abbott Laboratories fell 16 cents to $51.99.

Description: The recalled bracelets and balls are made of stretchy, rubber material with hard plastic, colorful balls attached at the end of the toy's arms. The toys were sold with orange, green, pink, purple and blue colored balls. The BoBo balls have a flashing lighted ball encased in the stretchy material.

Albee Baby Recalls C & T International/Sorelle Brand "Prescott" Cribs Due to Entrapment, Suffocation and Fall Hazards
Re-labeled Simplicity Cribs Contain Recalled Mattress Support Frames

Name of Product: Sorelle brand "Prescott" fixed-sided cribs
Units: About 130

Manufacturer: Simplicity Inc. (firm is no longer in business)
Hazard: These cribs are re-labeled fixed-sided Simplicity cribs that contain tubular metal mattress-support frames recalled in April 2010. The mattress support frames can bend or detach, causing part of the mattress to drop, creating a space into which an infant or toddler can roll and become wedged, entrapped or fall out of the crib.
Incidents/Injuries: In the April 2010 Simplicity recall, CPSC reported the death of a one-year-old child from Attleboro, Mass. who suffocated when he became entrapped between the crib mattress and the crib frame. In addition, CPSC has received reports of 29 incidents involving the Simplicity cribs where the cribs collapsed due to the metal mattress support frame detaching or bending. These include one child entrapment that did not result in injury and one child who suffered minor cuts when his head struck the broken mattress support bar. CPSC has received one report of a consumer who, in April of 2010, removed the Sorelle Prescott label from the crib and found a Simplicity crib label underneath. (The consumer purchased the crib in July of 2009, prior to the Simplicity mattress support recall.)
Description: These are full-sized fixed-sided cribs sold in an oak finish, as 3-in-1 or 4-in-1 convertible cribs. "Sorelle Furniture" along with the company's address, the crib's model number and a manufacturer's code are printed on a label attached to the headboard or footboard.
Sold at: This recall is limited to Sorelle "Prescott" cribs sold online by AlbeeBaby.com between July 2009 and October 2009 for between $180 and $210.
Manufactured in: China.
Remedy: Consumers should immediately stop using the recalled cribs and contact Albee Baby for a replacement crib, store credit or refund. C&T International/Albee Baby is attempting to directly contact known consumers who purchased the recalled crib online from July 2009 through October 2009. In the meantime, find an alternate, age appropriate, safe sleeping environment for the child, such as a bassinet, play yard or toddler bed.

Important Message from CPSC:
CPSC reminds parents not to use any crib with missing, broken or loose parts. Make sure to tighten hardware from time to time to keep the crib sturdy. When using a drop-side crib, parents should check to make sure the drop side or any other moving part operates smoothly. Always check all sides and corners of the crib for parts separating. Disengagements can create a gap and entrap a child. In addition, do not try to repair any side of the crib. Babies have died in cribs where repairs were attempted by caregivers. Crib age is a factor in safety. At a minimum, CPSC staff recommends that you not use a crib that is older than 10 years. Many older cribs may not meet current voluntary standards and can have numerous safety problems.

Name of Product: Chuck E Cheese’s Light-up Rings and Star Glasses
Units: About 1,100,000 Light-up Rings and 120,000 Star Glasses
Importer: CEC Entertainment, Inc., of Irving, Texas
Hazard: If crushed or pulled apart, the plastic casing can break into small pieces and possibly expose the batteries, posing an ingestion hazard to children. If ingested, the batteries may be damaging to either the stomach, intestine, esophagus or nasal mucus membrane.
Incidents/Injury: There have been two reported incidents involving the Light-Up Rings. One involved a child swallowing a battery, the other involved a child inserting a battery into his nostril. There are no reported incidents involving the Star Glasses.
Description:
- Light-Up Rings - The rings were distributed as part of a promotional product offering or during parent-teacher association conventions. The ring measures 1 1/8 inches across and is made of plastic with a black elastic band. The ring comes in several colors – blue, green, purple, yellow, and pink. The back of the ring is fastened either with screws or glue.
- Star Glasses - The glasses were distributed as part of a birthday package. The glasses measure about 5 1/2 inches across by 2 1/2 inches tall and are made of red translucent plastic and have the words Chuck E. Cheese’s painted on the side.
Sold at: Chuck E Cheese’s Restaurants from April 2009 through June 2010 (Light-up Rings) and April through August 2010 (Star Glasses).

CPSC Urges Parents to Inspect and Secure TVs, Furniture, and Appliances in Child-Proofing Efforts
On average, one child dies every two weeks due to tipovers
WASHINGTON, D.C. - Many parents and caregivers may not be aware that one of the top hidden hazards in the homes where young children live or visit is unsecured and unstable TVs, furniture and appliances. Today, the U.S. Consumer Product Safety Commission (CPSC) is urging families to take a moment to inspect and secure these items to prevent any more tragedies.
Between 2000 and 2008, CPSC staff received reports of nearly 200 tipover related deaths involving children eight years old and younger. Nearly all of these fatalities (93%) involved children five years old and younger.
More than 16,000 children five years old and younger were treated in emergency rooms
because of injuries associated with TVs, furniture, and appliance tipovers according to CPSC staff’s most recent estimates from 2006.
“Large TVs and unstable furniture can be a deadly combination. Taking simple, low-cost steps to secure furniture and TVs can save lives,” said CPSC Chairman Inez Tenenbaum. “Parents need to know about this hidden danger and take action now.”
Typically, injuries and deaths occur when children climb onto, fall against or pull themselves up on television stands, shelves, bookcases, dressers, desks, chests and appliances. In some cases, televisions placed on top of furniture will tip over and cause a child to suffer traumatic and sometimes fatal injuries.
To help prevent tip-over hazards, the CPSC offers the following safety tips:
- Furniture should be stable on its own. For added security, anchor chests, dressers, TV stands, bookcases and entertainment units to the floor or attach them to a wall.
- Place TVs on a sturdy, low-rise base. Avoid flimsy shelves.
- Push the TV as far back on its stand as possible.
- Place electrical cords out of a child’s reach and teach kids not to play with them.
- Keep remote controls and other attractive items off the TV stand so kids won’t be tempted to grab for them and risk knocking the TV over.
- Make sure free-standing ranges and stoves are installed with anti-tip brackets.
This year, the CPSC is intensifying its outreach efforts by partnering with numerous organizations, including clinics and second-hand stores, to disseminate a two minute PSA (transcript) including a powerful testimonial of a parent who lost her two-year-old in a TV tipover incident and a poster. In addition, consumers will be able to stream a 20 second version (transcript) of the PSA through October 23, 2010 by texting TVFALL to 878787.

Description: The wall hook sets were sold in three models: model numbers 74641 and 74642, constructed of five separate wooden letters spelling out the word “S T U F F” and model number 65262 consisting of four separate, different colored wooden paws. Model number 74641 has a Purple S, a Red T, an Orange U, a Green F and a Pink F. Model number 74642 has an Olive S, a Blue T, a Yellow U, a White-striped F, and a Blue F. Attached to each letter is a painted metal coat hook with a painted wooden ball at the end. Each letter is approximately 4 inches long, 3/4 inches wide and 9 ¾ inches tall. Model number 65262 has four separate paw-shaped wooden pieces of different colors -- red, blue, green and pink. Attached to each paw is a wooden peg with a wooden ball at the end. The “S T U F F” wall hook sets have a label on the back of the letter “S” that includes the models 74641 or 74642. The Paw wall hook sets have the model number 65262 on the outside of the box.

Deaths prompt CPSC, FDA warning on infant sleep positioners
WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission (CPSC) and the U.S. Food and Drug Administration (FDA) today warned consumers to stop using infant sleep positioners. Over the past 13 years, CPSC and the FDA have received 12 reports of infants between the ages of 1 month and 4 four months who died when they suffocated in sleep positioners or became trapped and suffocated between a sleep positioner and the side of a crib or bassinet.
Most of the infants suffocated after rolling from a side to stomach position. In addition to the reported deaths, CPSC has received dozens of reports of infants who were placed on their backs or sides in sleep positioners, only to be found later in potentially hazardous positions within or next to the sleep positioners.
“The deaths and dangerous situations resulting from the use of infant sleep positioners are a serious concern to CPSC,” said CPSC Chairman Inez Tenenbaum. “We urge parents and caregivers to take our warning seriously and stop using these sleep positioners, so that children can have a safer sleep.”
The two main types of infant sleep positioners are flat mats with side bolsters or inclined (wedge) mats with side bolsters.
Both types of sleep positioners typically claim to help keep infants on their backs and reduce the risk of Sudden Infant Death Syndrome (SIDS). The FDA has never cleared an infant sleep positioner to prevent or reduce the risk of SIDS. In addition, CPSC and the FDA are unaware of any scientific studies demonstrating that infant positioners prevent SIDS or are proven to prevent suffocation or other life-threatening harm.
“To date, there is no scientifically sound evidence that infant sleep positioners prevent SIDS,” said Dr. Joshua Sharfstein, FDA Principal Deputy Commissioner and a pediatrician. “We want to make sure parents, health care professionals, and childcare providers understand the potential risk of suffocation and stop using infant sleep positioners.”
Sleep positioners also typically claim to do one or all of the following: aid in food digestion to ease colic or the symptoms of gastroesophageal reflux disease (GERD); and prevent flat head syndrome (plagiocephaly). In light of the new safety data, FDA believes any benefit from using these devices to ease GERD or prevent plagiocephaly is outweighed by the risk of suffocation.
CPSC and the FDA are warning parents and child care providers to:
- STOP using sleep positioners. Using a positioner to hold an infant on his or her back or side for sleep is dangerous and unnecessary.
- NEVER put pillows, infant sleep positioners, comforters, or quilts under a baby or in a crib.
- ALWAYS place an infant on his or her back at night and during nap time. To reduce the risk of SIDS, the American Academy of Pediatrics recommends placing infants to sleep on their backs and not their sides.
The American Academy of Pediatrics does not support the use of any sleep positioner to prevent SIDS.
Manufacturers of infant sleep positioners with medical claims that have not been reviewed by the FDA should stop marketing those products until they submit, and the FDA clears, appropriate premarket review submissions, including necessary safety and effectiveness data.
FDA has informed manufacturers of cleared devices of the agency's serious concern and has requested that they submit clinical data showing the benefits of their products outweigh the risk of suffocation or other serious harm.
Prompt reporting of adverse events can help the FDA and CPSC identify and better understand the risks associated with infant sleep positioners. Incidents/Injuries: CPSC and Fisher-Price are aware of 10 reports of incidents resulting in injury. Six of the incidents required medical attention after young girls, ages two to three years old, fell against or on the protruding disc-shaped and D-shaped pretend key.
Description: This recall involves the Fisher-Price Trikes and Tough Trikes toddler tricycles with model numbers listed in the chart below and that have either a disc-shaped or D-shaped pretend key. The model numbers are located under the seat in the storage compartment. The trikes are intended for children 2 to 5 years of age. The pretend keys are located about 3 inches in front of the seat and protrude at least 5/8 inches above the trike's body. The trikes manufactured after June 16, 2010 are not included in this recall. These trikes have a modified key in a flattened D shape (see picture below) and a manufacturer run number higher than 1670Q2. The run number indicates the trike was manufactured on the 167th day of 2010 or on June 16, 2010. The run number is found under the seat below the model number.
Sold at: Mass merchandise stores nationwide from January 1997 through September 2010 for about $25.
