Wooden Toys Recalled by Earth Friendly Due To Choking Hazards

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Wooden Toys

Units: About 1,000

Importer: Earth Friendly LLC, of Beaverton, Ore.

Hazard: Small parts can detach and break from the toy, posing a choking hazard to young children. In addition, the size of the rattle handle violates voluntary rattle standards.

Incidents/Injuries: None reported.

Description: This recall involves three models of wooden toys. Moee the car, Cubby the stackable bear and the Bell rattle. The toys are painted in glossy red, orange, green, red, black and yellow.

Sold at: Toy specialty stores in Alaska, California, Colorado, Montana, Nevada, Oregon, and Washington from April 2008 through September 2008 for between $12 and $22.

Remedy: Consumers should immediately take the recalled toys away from children and contact Earth Friendly to exchange or refund the product.

CPSC Provides Safety Tips for a Safe "Trick or Treat"

WASHINGTON, D.C. – The spookiest night of the year is around the corner. Kids everywhere are choosing their favorite costume and looking forward to their favorite treats. As parents and caregivers prepare for this celebration, the Consumer Product Safety Commission (CPSC) reminds them to keep safety in mind, so that no child is haunted by Halloween-related injuries. Hidden dangers associated with costumes, treats, and decorations can be easily prevented.

CPSC reports that the most serious Halloween-related injuries involve burns from flammable costumes and decorations, including ignition from open flames, such as candles and Jack O’Lanterns. Other incidents have involved lacerations from carving pumpkins and trips/falls while walking, particularly after dark.

“Our major concern is still the use of home-made costumes that are not flame resistant,” said CPSC Acting Chairman Nancy Nord. “Parents making their children’s costumes should use inherently flame-resistant fabrics, such as nylon and polyester. Costumes should fit well and not drag on the ground to guard against trips and falls.”

CPSC helps keep children safe by enforcing the Flammable Fabrics Act and by recalling products at Halloween and throughout the year that can cause injury.

Follow these safety tips to ensure this year’s holiday is a safe one:

Costumes

When purchasing costumes, masks, beards and wigs, look for flame-resistant fabrics such as nylon or polyester, or look for the label "Flame Resistant." Flame-resistant fabrics will resist burning and should extinguish quickly. To minimize the risk of contact with candles and other fire sources, avoid costumes made with flimsy materials and outfits with big, baggy sleeves or billowing skirts.

Purchase or make costumes that are light, bright and clearly visible to motorists.

For greater visibility during dusk and darkness, decorate or trim costumes with reflective tape that will glow in the beam of a car's headlights. Bags or sacks also should be light-colored or decorated with reflective tape. Reflective tape is usually available in hardware, bicycle and sporting goods stores.

Children should carry flashlights to see and be seen.

Children should wear well-fitting, sturdy shoes. Oversized high heels are not a good idea.

Tie hats and scarves securely to prevent them from slipping over children's eyes and obstructing vision.

If your child wears a mask, make sure it fits securely, provides adequate ventilation, and has eye holes large enough to allow full vision.

Swords, knives and similar costume accessories should be made of soft, flexible materials.

Supervise pumpkin carvings to avoid lacerations.

Treats

Warn children not to eat any treats until an adult has examined them carefully for evidence of tampering.

Carefully examine any toys or novelty items received by trick-or-treaters under three years of age. Do not allow young children to have any items that are small enough to present a choking hazard or that have small parts or components that could separate during use and present a choking hazard.

Decorations

Keep candles and Jack O' Lanterns away from landings and doorsteps where costumes could brush against the flame.

Indoors, keep candles and Jack O' Lanterns away from curtains, decorations and other combustibles that could catch fire. Do not leave burning candles unattended.

Remove obstacles from lawns, steps and porches when expecting trick-or-treaters.

Indoors or outside, use only lights that have been tested for safety by a recognized testing laboratory, such as UL. Check each set of lights, new or old, for broken or cracked sockets, frayed or bare wires, or loose connections. Discard damaged sets.

Don't overload extension cords.

Gaming System Batteries and Recharging Station Recalled by LeapFrog Due to Burn Hazard

Name of Product: Rechargeable Batteries and Recharging Station for Didj Custom Gaming System

Hazard: The rechargeable batteries included with the Recharging Station can overheat if the gaming system is placed into the recharging base upside down, posing a burn hazard to consumers.

Incidents/Injuries: LeapFrog has received 12 reports of the rechargeable batteries overheating, including one report of a minor hand burn.

Description: The Recharging Station comes with two rechargeable batteries, an AC adapter, and a recharging base. The recharging base is white with a green indicator light on the bottom. The Recharging Station for Didj was sold as item number 30676, which is printed on the box and on the bottom of the recharging base.

CPSC and Carter’s Advise Parents of Rashes Associated with Heat Transferred, or “Tag-less,” Labels

WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission (CPSC) and Carter’s, Inc., of Atlanta, Georgia, are advising parents and caregivers that they have received reports that a small percentage of babies and infants have developed rashes on the upper back after wearing Carter’s clothing with heat-transferred, or “tag-less,” labels.

This advisory applies to Carter’s Fall 2007 product line. The Fall 2007 line utilizes a label on the inside back of the garment that has a raised surface with a solid, rather than a stenciled, background. This advisory does not apply to previous and current product lines, which utilize labels with stenciled backgrounds.

The garments, which were made in various countries, were sold at Carter’s own retail stores and at department and national chain stores.

If your child develops a rash on the upper back after wearing garments that have a “tag-less” label with a solid background, you should stop using these garments. If the rash persists or worsens, you should contact your pediatrician. Shimano American Corp. Recalls Wheels for Bicycles; Flat Tire Can Pose Injury Risk to Riders

Name of Product: Shimano Dura Ace Carbon Clincher Wheels for Bicycles

Hazard: Rim surface and spoke hole plugs on the wheel can cause a puncture to the inner tube, resulting in a flat tire. This can cause the rider to lose control and fall.

Incidents/Injuries: None reported.

Description: This recall involves the Shimano Dura Ace Carbon Clincher Wheel Sets with model number WH-7850 C24CL sold for road racing bicycles. The rim has labels that read “Shimano WH-7850”, “Dura Ace”, and “Carbon 1380.” These wheels are intended for use with high end adult road racing bicycles.

Sold at: Bicycle specialty stores and dealers nationwide from April 2008 through August 2008 for about $1,300.

Manufactured in: Malaysia

Remedy: Consumers should stop using the bicycles with the recalled wheels, remove the wheels, and return them to their local bike dealer or retailer for a free inspection and repair.

Hazard: The multi-purpose lighters fail to meet federal safety standards because they might not contain a child-resistant mechanism, posing a potential fire and burn hazard to young children.

Rage Wireless Guitars Used with Popular Video Gaming System Recalled Due to Chemical Burn Risk

Name of Product: Rage Wireless Guitars

Hazard: A circuit board defect can cause AA batteries used in the guitar to leak if the batteries are installed incorrectly, posing a risk of chemical burns to consumers.

Incidents/Injuries: Performance Designed Products has received one report of a burn to a consumer.

Description: The Rage Wireless Guitar controller is designed to be used on the Nintendo Wii system. The guitar is blue or white and is 31 inches long. The guitar contains battery-operated LED-lighted fret buttons that go up the neck of the guitar.

Sold at: Mass merchandisers and specialty retailers nationwide from June 2008 through September 2008 for between $40 and $60.

Defects Identified by CPSC Early Warning System Prompt Crib Warning to Parents

CPSC to consider rulemaking to address crib defects

WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission (CPSC) is urging parents and caregivers to closely inspect the hardware and stability of their cribs to ensure all parts are in place and secure. The warning by the CPSC comes after the agency’s Early Warning System has identified concerns with the durability of cribs, especially those with drop sides that can disengage and lead to dangers of entrapment and strangulation.

Since the creation of the CPSC Early Warning System in fall of 2007, the agency has conducted five crib recalls where the hardware was broken, missing or otherwise failed to function. Cribs with drop sides are the type most likely to experience hardware problems. They contain more moving parts and have more non-rigid connections than static, or non-drop side cribs. In many cases the drop side corners disengage from the tracks located on the crib ends, or safety stops become nonfunctional permitting the drop side to detach from the crib. These types of defects are often undetected by parents or caregivers and can worsen when the baby pushes or leans against the side of the crib.

“The CPSC is committed to making sure a baby’s sleep environment is as safe as possible,” said Acting Chairman Nancy Nord. “It is that ongoing commitment that is driving the agency to explore new crib requirements and educate the public of the dangers associated with some cribs.”

Safety Tips for all cribs:

Parents should not use any crib with missing, broken or loose parts.

Hardware should be inspected from time to time and tightened to keep the crib sturdy.

When using a drop side crib parents should check to make sure the drop side or any other moving part operates smoothly on its track.

Always check all sides and corners of the crib for disengagement. Any disengagement can create a gap and entrap a child.

Do not try to repair any side of the crib without manufacturer approved hardware or with tape, wire or rope.

Putting a broken side up against the wall does not solve the problem and can often make it worse.

While the mandatory and voluntary crib standards have succeeded in preventing many deaths and injuries, the agency staff believes the performance requirements can be strengthened to deal with the problems identified by the Early Warning System. Therefore, agency staff will be recommending that the Commission vote to issue an Advanced Notice of Proposed (ANPR) rulemaking to examine and assess potential design and durability issues and possible mandatory performance requirements to prevent future entrapments and strangulations to children. If approved, the ANPR will seek input and information about hardware systems, other hardware issues, assembly and instructional problems and wood quality/strength issues for cribs with both stationary and drop side construction.

WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission (CPSC), in cooperation with Delta Enterprise Corp., of New York, New York., is announcing the voluntary recall to replace missing safety pegs involving 985,000 drop side cribs. Failure to use or install safety pegs can cause an entrapment and suffocation risk to infants and toddlers.

When the safety pegs in the base of each leg of the crib are missing from the lower track, the crib locks can disengage and detach if lowered below the peg hole, creating a hazardous gap. This gap can lead to the entrapment and suffocation of an infant or toddler.

CPSC staff is aware of a death of an 8-month-old child who became entrapped and suffocated when the drop side of the crib detached in a reassembled crib where the safety pegs were not installed. CPSC is also aware of two entrapments and nine disengagement incidents in cribs where the safety pegs were missing.

The recall involves all Delta cribs manufactured in Taiwan or Indonesia, with the “Crib Trigger Lock with Safety Peg” drop side hardware design. These model numbers and country of origin can be located on the mattress support board label: 4320, 4340, 4500, 4520, 4530, 4532, 4540, 4542, 4550, 4551, 4580, 4600, 4620, 4624 (production dates 01/06 thru 11/07), 4640, 4660, 4720, 4735, 4742, 4750 (production dates 01/95 thru 12/00), 4760, 4770, 4780, 4790, 4820, 4840, 4850, 4860, 4880, 4890, 4892, 4900, 4910, 4920, 4925-2, 4925-6, 4930, 4940, 4943, 4944, 4947, 4948, 4949, 4950, 4958, 4963, 4968, 4969, 4980.

The recalled cribs have date codes ranging from 1995 through December 2005 and one model (4624) was made in 2007. The model numbers are located on the top of the mattress support board.

The cribs, which were made in Taiwan and Indonesia, were sold at major retailers, including Walmart, Kmart and Target.com from January 1995 through September 2007 for about $100. These cribs have also been found to be sold secondhand. Cribs currently sold at retail are not involved.

CPSC and Delta urge parents and caregivers to immediately stop using cribs that are missing a safety peg on any leg of the crib and contact Delta to receive a free, easy-to-install repair kit. The kit will include safety pegs in a bold color and warning labels to be affixed to the mattress board. Consumers with cribs that have all safety pegs installed may continue to use the cribs; however, CPSC recommends that these consumers contact Delta for the repair kit for future assembly purposes. The crib’s drop side can detach when the spring peg is not engaged, which can cause an entrapment and suffocation risk to infants and toddlers.

If the spring pegs in the base of each leg of the crib are pushed back into the lower track, the pegs can become nonfunctional. This can cause the drop side to detach from the crib and create a gap that can entrap an infant or toddler.

CPSC staff is aware of a death of an 8-month-old child who suffocated when the drop side of his crib detached. CPSC is also aware of one child entrapment and one incident in which the drop side detached due to a disengaged spring peg.

The recall involves all Delta drop side cribs manufactured prior to 2006 that use “Crib Trigger Lock with Spring Peg” drop side hardware design. The recalled model numbers and country of origin include: 4340, 4343, 4520, 4600, 4620, 4624, 4625, 4629, 4660, 4665, 4720, 4750, 4751, 4850, 4855, 4857, 4880, 4920, 4925-2, 4925-2B, 4925-6, 4980, and 8605. The model number, country of origin and manufacture dates are located on top of the mattress support board. The recalled cribs have Delta’s name and address on the mattress support boards and Delta’s logo on the teether rail.

The cribs, which were made in China, were sold at major retailers nationwide from January 2000 through January 2007 for between $33 and $200. Cribs currently sold at retail are not involved.

Infant Death Prompts Recall of Convertible Cribs by Playkids USA; Crib Poses Entrapment and Suffocation Hazards

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission (CPSC), in cooperation with Playkids U.S.A. of Brooklyn, N.Y. is announcing today a voluntary recall of about 2,000 convertible cribs. The sides of the convertible crib are made of a mesh that expands, creating a gap between the mattress and the side through which an infant can slip. This poses suffocation and entrapment hazards for young children.

CPSC is aware of the death of a 5-month-old child on August 31, 2008 in Brooklyn, N.Y. The child became entrapped between the mattress and the stationary side rail of the convertible crib and suffocated.

This recall involves the Playkids U.S.A. convertible crib/playpen/bassinet/bed with model number PLK-909. “Playkids U.S.A.” can be found on the packaging and on a label sewn into the side of the crib. The model number can be found on the packaging. The convertible cribs have a drop side rail, a stationary side rail, a canopy assembly, and a bassinet. Hazard: The walkers violate the baby walker voluntary standard and can fit through a standard doorway and are not designed to stop at the edge of a step. Babies using these walkers can be seriously injured or killed.

Description: The recalled walkers were sold for babies 6 months and older. The walkers were sold in the following colors: pink, red, green, blue, and ivory. The walkers have an activity tray with a steering wheel and other toys. “My Way Corp” is printed on a sticker on the front of the walker.

Sold at: Independent discount stores in Puerto Rico from November 2004 through March 2008 for between $18 and $25.

Toy Boats Recalled Due to Burn Hazard

Hazard: The two battery terminals can come into contact with each other, causing the battery to overheat, posing a burn hazard to consumers.

Description: The recalled toy speed boats are lightweight plastic toy boats supported by an inflatable hull with “outboard” motors on them. The motor uses two AA batteries. The toy boat measures about 12 inches long by 8 inches wide and comes in various colors and designs.

Sold at: Dollar General nationwide from March 2008 through July 2008 for $3 each.

Hazard: The blaster’s plunger can pull the user’s skin during firing of the toy blaster resulting in injury to the face, neck, and/or chest.

Incidents/Injuries: Hasbro and CPSC have received 46 reports of injury to children aged 4 to 12, resulting in bruising, abrasions, pinching/pinch marks, blood blisters and welts.

Description: This recall involves the Nerf N-Strike Recon CS-6 Blasters for children age 6 and up. The toy blaster is yellow with a black handle and orange plunger, trigger, and reload clip. The word “NERF” in black lettering is on both sides of the blaster and the word “ARMED” is indented on the orange plunger. “RECON CS-6” is on the gray cocking mechanism. There are five interchangeable parts including the shoulder stock, the flip-up sight, barrel extension, quick re-load clip and dual-mode light beam. Model number 63552 and UPC codes 653569272021 and 653569311218 can be found on the packaging. Only blasters with an exposed orange plunger are included in this recall.

Sold at: Wal-Mart, Target, Toys “R” Us, discount stores and toy stores nationwide from November 2007 through August 2008 for about $20.

CPSC and Chance Rides Manufacturing Announce Recall to Inspect and Repair YO-YO Amusement Rides

WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Chance Rides Manufacturing Inc. (CRM), of Wichita, Kansas is voluntarily recalling to inspect and repair about 85 YO-YO amusement rides across the country.

CPSC has received reports of two incidents involving the YO-YO ride resulting in injuries to children and adults, including scraped knees and back strains. On May 16, 2008, 23 riders of a YO-YO ride in Angels Camp, Calif. fell to the ground when the sweep arms suspending the circular swinging chairs they were riding in broke free from the ride’s center cylinder rod. The inspection program will focus on preventing future failures.

CRM is offering ride owners and state safety officials inspection/repair kits and new inspection and maintenance guidelines. To receive the inspection/repair kit or for more information, owners and state safety officials can contact CRM at (800) 242-6231 between 8 a.m. and 5 p.m. CT Monday through Friday.

CPSC is working with state regulators and insurance providers to ensure that these safety inspections and modifications are conducted. While CPSC has jurisdiction over mobile amusement rides and conducts incident investigations, states and local communities are responsible for inspections and oversight of rides such as the YO-YO.

Distributor: Backyard Play Systems LLC, of Monroe, Mich.

Hazard: Some of the gliders were shipped with assembly instructions that did not inform consumers to tighten all lock nuts during assembly, including those attached by the manufacturer. As a result, some lock nuts were not fully fastened during assembly which could cause the glider to detach, posing a fall hazard to children.

Incidents/Injuries: Backyard Play Systems has received one report of a lock nut falling off. No injuries have been reported.

Description: This recall involves outdoor play set gliders designed to hang from home play equipment. The green plastic gliders were sold as an accessory to home play equipment marketed under the brand names Heartland Play Systems, Yardline Play Systems and Backyard Play Systems.

Sold by: Backyard Buildings and More and Lowe’s stores nationwide, and online at Costco.com, BettyMills.com, and BackyardBuildings.com from February 2008 through July 2008 for about $100.

Remedy: Consumers should immediately stop using the gliders until all lock nuts are securely fastened. Backyard Play Systems is notifying all customers directly and is providing written instructions indicating the location of all lock nuts that must be securely fastened.
