Infant Death Prompts Recall of Ring Slings Made by Sprout Stuff Due to Suffocation Risk
WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission (CPSC), in cooperation with Sprout Stuff, of Austin, Texas, is announcing the recall of about 40 Sprout Stuff infant ring slings. CPSC advises consumers to immediately stop using these slings due to a risk of suffocation to infants.
CPSC and Sprout Stuff are aware of one report of a death of a 10-day-old boy in the recalled sling in Round Rock, Texas in 2007.
The Sprout Stuff infant ring sling is fabric/natural muslin and comes with or without a shoulder pad. The sling is worn by parents and caregivers to carry a child up to two years of age. “Sprout Stuff” is printed on the back side of the tail’s hem.
Sprout Stuff sold the recalled infant slings, which were made in the United States, directly to consumers between October 2006 and May 2007 for between $35 and $45. Sprout Stuff is directly contacting known purchasers of the recalled infant slings.
Consumers should immediately stop using the recalled slings and contact Sprout Stuff to return the sling for a full refund. Additional Message from CPSC: On March 12, 2010, CPSC issued a warning about sling carriers for babies. Slings can pose two different types of suffocation hazards to babies. In the first few months of life, babies cannot control their heads because of weak neck muscles. The sling’s fabric can press against an infant’s nose and mouth, blocking the baby’s breathing and rapidly suffocating a baby within a minute or two. Additionally, where a sling keeps the infant in a curled position bending the chin toward the chest, the airways can be restricted, limiting the oxygen supply. The baby will not be able to cry for help and can slowly suffocate. CPSC has determined that a mandatory standard is needed for infant sling carriers. While a mandatory standard is being developed, CPSC staff is working with ASTM International and concerned companies such as Infantino to quickly develop an effective voluntary standard for slings. There currently are no safety standards for infant sling carriers.

Hazard: The fabric covering the stacking ring’s center pole can come apart at the seam exposing the foam material inside. The foam material poses choking and aspiration hazards to young children.
Incidents/Injuries: The firm has received one report of a 10-month-old child mouthing the toy’s foam material. No injuries have been reported.
Description: This recall involves Animal Crackers Giant Stacking Toys. The multi-colored toy has five plush stacking rings that stack and rest on a stationary turtle base. The rings represent different animals including a frog, dog, cat, rabbit, mouse and chick. A fish is on top of the center pole. Each ring makes a different sound: rattle, jingle, crinkle and squeak. “One Step Ahead” is printed on a white tag attached to the toy. The toy measures about 19 inches high and 10 inches wide when assembled.

Description: This recall involves the Bayside youth bed: the Pirates of the Caribbean Twin Trundle Bed. The preassembled headboard has a brown wood base with decorative carvings, three open storage compartments, a storage bin and a mast. The storage bin located in the headboard, measures approximately 39 inches long, 6 1/2 inches wide and 24 inches deep. The "Pirates of the Caribbean" trademark is embossed on a metal plate inside the headboard. The name and address of the manufacturer, model number, manufacture date and "Made in China" is printed on a label affixed to the inside of the mattress frame side rails.

The product measures six inches in diameter and is composed of four blue plastic "hubs", six white plastic "wires" and twelve multi-colored beads that slide along the wires. The model number and date code are printed on the bottom of the packaging and the date code is also inside of the blue hubs (see picture below). Beados that have a date code different than 02910 or have a date code of 02910 and an inspection sticker on the bottom of the packaging and a black mark by the date code inside the blue hub (see picture below) are not included in this recall.

Hazard: The cribs’ drop sides can malfunction, detach or otherwise fail, causing part of the drop side to fall out of position, creating a space into which an infant or toddler can roll and become wedged or entrapped, which can lead to strangulation or suffocation. A child can also fall out of the crib. Drop-side incidents can also occur due to incorrect assembly and with age-related wear and tear.
Incidents/Injuries: CPSC has received 30 reports of drop sides that have malfunctioned or detached. Two children became entrapped between the drop side and the crib mattress but were freed without injury. Two other children fell out of their cribs when the drop side malfunctioned and one child sustained scratches due to the fall.

Important Message from CPSC: CPSC reminds parents not to use any crib with missing, broken, or loose parts. Make sure to tighten hardware from time to time to keep the crib sturdy. When using a drop-side crib, parents should check to make sure the drop side or any other moving part operates smoothly. Always check all sides and corners of the crib for disengagement. Disengagements can create a gap and entrap a child. In addition, do not try to repair any side of the crib. Babies have died in cribs where repairs were attempted by caregivers. Age is factor in the safety of any crib. At a minimum, CPSC staff recommends that you not use a crib that is older than 10 years. Many older cribs may not meet current voluntary standards and can have numerous safety problems.

Fourteen other incidents involved no injuries. Incidents/Injuries: CPSC and Delta have received 57 reports involving drop sides that have malfunctioned or detached, resulting in three entrapments, including two reports of bruises and one child who fell out of the crib. Additionally, CPSC is aware of 19 reports in which stabilizer bars were installed upside-down, resulting in 10 mattress platform collapses. Two children were entrapped but freed without injury and one child sustained scratches.
Description: This recall involves Delta drop-side cribs with three different types of drop-side hardware and Delta cribs with wooden stabilizer bars that support the mattress platform. Manufacturer: Child Craft Industries, Inc. (Child Craft ceased operations in July of 2009 and sold the "Child Craft" name to Foundations Worldwide Inc., of Medina, Ohio) Foundations Worldwide, Inc. neither manufactured nor sold any of the cribs included in this recall.

Hazard: The cribs' stationary side can be assembled upside-down but still appear to be assembled correctly. If assembled upside-down, the crib side contains a hazardous five-inch gap at the top of the crib. Infants or toddlers can become entrapped in this gap (as depicted in the photo below), which can lead to entrapment, strangulation or other injuries.
Incidents/Injuries: CPSC has received four reports of children becoming entrapped between the dowel and the crib's stationary front side. In two of those reports, the child was trapped by his/her head and was in danger of being strangled. In the other two reports, the child was trapped by his/her arm. This hazard can occur on both the front and back sides of the crib.
Description: All Child Craft brand cribs with the foot top dowel (as shown in the photos below) including, but not limited to, the following models:

Remedy: The CPSC staff urges parents and caregivers to examine these cribs immediately and determine whether they have been assembled properly. The best way to do this is to measure the gap between the front and back sides of the crib and the dowel that runs parallel to the sides. The gap should be no more than two and three eighths inches (2 3/8"), as shown in the images below. If the gap is greater than 2 3/8", the side has been installed upside-down and needs to be re-assembled. Another indicator that the side has been installed improperly is the appearance of the barrel nut, as is visible in the image above showing an incorrect assembly. The barrel nut should be at the bottom of the side, not the top. If either of these conditions are present, the side of the crib has been installed upside-down and needs to be re-assembled so that the barrel nut is not visible from the top and the gap between the dowel and the side is no greater than two and three eighths inches (2 3/8"). Consumers with these cribs should contact Foundations Worldwide, Inc., to obtain instructions and decals to affix to the stationary sides of the crib to ensure proper assembly in the future.

Industry Launches New Initiative to Secure Drop Sides with Free Immobilization Devices
WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission (CPSC), with the cooperation of seven firms, is announcing voluntary recalls of more than two million cribs to address drop-side hazards and other hazards that affect the safety of young children. The recalling firms are providing consumers with free repair kits to immobilize the drop sides or other remedies. Do not attempt to fix these cribs with homemade remedies.
The drop-side and fixed-side crib recalls announced today are of units manufactured between 2000 and 2009 by the companies listed below. Consumers should contact these firms directly for the appropriate remedy:
“Cribs should be the safest place in the home for infants and toddlers,” said CPSC Chairman Inez Tenenbaum. “CPSC is committed to addressing the hazards with cribs and to restoring parents’ confidence that their child will have a safe sleep.”

CPSC staff is also working on a new mandatory standard to make cribs safer, which is targeted for completion in 2010.
The Juvenile Products Manufacturers Association (JPMA) has also launched a new crib safety initiative. The listed manufacturers are providing free drop-side crib immobilization kits to prevent the drop side from detaching, plus replacement hardware and assembly instructions for cribs manufactured by participating firms. These materials are available free to any consumer by request.
The firms involved in today’s recalls are providing immobilization devices or other remedies as part of JPMA’s crib safety initiative. Consumers can visit JPMA’s website, www.cribsafety.org, for a list of participating manufacturers and for downloadable materials about ensuring that children have a safe sleep.
The immobilization devices, which will be available in the next few weeks, should be attached to keep the drop side from detaching from the cribs. Immobilization devices are not a solution for cribs with broken or damaged drop-side hardware. If your drop-side hardware is broken, contact the manufacturer for an alternative remedy.
CPSC issued a warning last month alerting parents and caregivers that there can be deadly hazards associated with drop-side cribs. Nine million drop-side cribs have been recalled over the past five years. CPSC staff has determined drop-side cribs generally have a tendency to be less structurally sound than cribs with four fixed sides.
Drop-side crib incidents can also occur due to incorrect assembly or age-related wear and tear. Age is a factor in the safety of any crib. At a minimum, CPSC staff recommends that you not use a crib that is older than 10 years. Many older cribs do not meet current voluntary standards and can have numerous safety problems.
The safest place for your baby to sleep is in a crib or bassinet depending on their age. If your crib has been recalled or it has missing, broken or loose parts, find an alternate safe sleep environment intended for a baby. If your baby is less than six months old and is not yet able to push up to his/her hands and knees, you can put your baby to sleep in a bassinet. Make sure your bassinet has not been recalled. Also, you can use a play yard.
Do not put additional bedding such as pillows, thick quilts, comforters or anything plush into your baby’s sleeping space. More babies die every year from suffocation in plush sleeping environments than from defective cribs. Always place your baby on his or her back to reduce the risk of Sudden Infant Death Syndrome (SIDS).

Baby Walkers Recalled by Suntech Enterprises Due to Fall Hazard
WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Name of Product: Baby Walkers
Units: About 8,400
Importer: Suntech Enterprises Inc., of Commerce, Calif.
Hazard: The recalled walkers can fit through a standard doorway and fail to have sufficient stair-fall protection to prevent falls down stairs. Babies using these walkers can be seriously injured or killed if they fall down stairs.
Incidents/Injuries: None reported.
Description: The baby walkers have a plastic frame supported by four wheels and eight brake pads. The walkers were sold in blue, pink, and green with a white activity tray and patterned, vinyl seat. Item number WK110 or WK112 is printed on the side of the packaging.
Sold at: Small juvenile product stores in California, Illinois, New York and Texas from January 2007 through December 2009 for between $25 and $30.

Remedy: Consumers should immediately stop using the walkers and return them to the store where purchased for a full refund.

Scope® Original Mint Mouthwash Recalled by Procter & Gamble Due to Failure to Meet Child-Resistant Closure Requirement

Units: About 35,000 bottles
Manufacturer: The Procter & Gamble Co., of Cincinnati, Ohio
Hazard: The mouthwash contains ethyl alcohol and certain bottles have malfunctioning child-resistant closures. Ethyl alcohol is toxic and can cause serious injury or death if ingested by children.
Incidents/Injuries: None reported
Description: This recall involves some bottles of Scope® Original Mint Mouthwash in 1 liter sizes. The recalled bottles have the number 4 on the bottom of the bottle. The bottles with the 4 on the bottom may not be child-resistant. Consumers can also attempt to twist the cap open. If the cap can be twisted off without squeezing the tabs on the cap, the package is not child-resistant.
Sold at: Drug stores, grocery stores and other retailers between January 2010 and June 2010 for about $4.

Remedy: Consumers should keep this product out of the reach of children. Consumers who purchased the product with the expectation that it would be in child-resistant packaging can contact Procter & Gamble for a full refund or a replacement coupon. The “Happy” charm bracelet is comprised of colorful beads on a small elastic band to which a metal charm in the shape of a butterfly, moon or sun is attached.
