Violation of Federal Mattress Flammability Standard Prompts Recall of Ottoman Bed Mattresses by PBteen

The following product safety recall was voluntarily conducted by the firm in cooperation with the CPSC. Consumers should stop using the product immediately unless otherwise instructed.

Name of Product: PBteen Ottoman Beds

Units: About 3,000

Importer: PBteen a division of Williams-Sonoma Inc., of San Francisco, Calif.

Hazard: The Ottoman Bed mattresses fail to meet the mandatory federal open flame standard for mattresses, posing a fire hazard to consumers.

Incidents/Injuries: None reported.

Description: This recall involves mattresses sold as part of PBteen Ottoman Beds with registration number CA -31586(TW). These are multifunctional ottomans that can be used to sit or sleep. The ottoman can be turned into a twin-sized bed. They were sold with a cover in stone, navy, ivory or pink. The ottoman cover has a tag that reads "PBteen" and a label with registration number CA-31586(TW).

Sold at: PBteen catalog and website from August 2008 through November 2009 for about $300.

Manufactured in: Taiwan

Remedy: Consumers should immediately contact PBteen to receive a free mattress cover that will bring the mattress into compliance with the flammability standard.

Bauer Recalls Children's Hockey Sticks Due to Violation of Lead Paint Standard

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission and Health Canada, in cooperation with the firm named below, today announced a voluntary recall of the following products. Consumers should stop using products immediately unless otherwise instructed.

Name of Product: Youth and Junior Hockey Sticks, Shafts, and Blades

Units: About 67,000 in the United States and 60,000 in Canada

Importer: Bauer Hockey Inc., of Greenland, N.H.

Hazard: Paint and decals on the sticks, shafts and blades contain excessive levels of lead, violating the federal lead paint standard.

Incidents/Injuries: None reported.

Description: The following hockey sticks, shafts and blades are involved in the recall:

The hockey and goalie sticks, shafts and blades come in various shapes, sizes and colors. The name "Bauer" and the model descriptions are on all of the sticks, shafts and blades. Most of the sticks also have the Nike symbol.

Junior player sticks and replacement shafts are each identified by the markings "JUNIOR," "52 Flex" or "JUNIOR Flex 52."

Youth player sticks are identified by the marking "YOUTH Flex 42" or "YTH Flex 42." These markings appear on all models on the narrow side of the stick shaft either near the top of the stick or near the blade.

Junior replacement blades fit only the Junior sticks and are identified by the markings "P92" or "PM9" followed by "JR", which appear on the narrow side of the blade near where the shaft and blade are joined.

Junior goal sticks are identified by the markings "P31 - JR - 22 ½" or "P31 JUNIOR - FLEX 52 - 22 ½" which appear on the narrow side of the stick shaft near the top of the stick.

Sold at: Sporting goods stores nationwide from February 2005 through March 2010 for about $80 to $200 for sticks, $30 for blades, and $40 to $90 for shafts.

Manufactured in: China

Remedy: Consumers should take the recalled sticks, shafts and blades away from children immediately and contact Bauer for a replacement or refund.

Gund Recalls to Replace Baby Books Due to Choking Hazard

Name of Product: Gund Baby Paperboard Books

Units: About 15,100 in the United States and 865 in Canada

Importer: Gund, of Edison, N.J.

Hazard: The styrofoam used to fill the book binding can detach, posing a choking/aspiration hazard to infants and young children.

Incidents/Injuries: Gund has received three reports of children mouthing the styrofoam that detached from the books. No injuries have been reported.

Description: This recall involves three miniature children's paperboard books with plastic handles designed as baby rattles. They include "Animals" (item number 059174), "Numbers" (item number 059175) and "Colors" (item number 059176). The books were also sold as a three-book set (item number 059173). The name "Gund" and the item number are located on the back of the books. The books measure 4 1/2 inches by 7 inches.

Sold at: Gift and specialty stores nationwide from January 2009 through March 2010 for about $8 and $20 for the set.

Manufactured in: China

Remedy: Consumers should immediately take the books away from young children and contact Gund for a free replacement product.

Le Hing Inc. Announces Recall of Baby Walkers Due to Fall Hazard

Name of Product: Baby Walkers

Hazard: The walkers can fit through a standard doorway and are not designed to stop at the edge of a step. Babies using these walkers can be seriously injured or killed.

Description: This recall involves children's Niko brand hooded sweatshirts with drawstrings through the hood. The garment-dyed sweatshirts were sold in mango, kiwi, rust, denim, pink, brown and periwinkle and in sized 2T through 5T. A tag that reads, "Niko's SPORTSLINE" is sewn on the back neck of the garment.

Sold at: Gift shops, children's stores, souvenir and resort stores in Delaware, Florida, Louisiana, New Jersey, Maryland and Michigan from June 2007 through January 2008 for about $8.

The sweater jackets have a drawstring through the waist that can pose an entanglement hazard to children. Incidents/Injuries: None reported.

Description: This recall involves two styles of girls’ jackets. The Shampoo brand jackets are denim with a pink hood and sleeves, and were sold in sizes 5/6, 7/8, 10/12 and 14/16. The Marci & Me brand sweater jackets are black or tan and were sold in sizes 7/8, 10/12 and 14/16. The tag at the neck reads “Shampoo” or “Marci & Me.”

Sold exclusively at: Burlington Coat Factory stores nationwide from September 2007 through September 2009 for between $11 and $13.

Description: This recall involves three styles of children hooded sweatshirts with drawstrings. The zippered sweatshirts were sold in sizes XS through 3XL. The HAY style sweatshirts were sold in black, navy, light grey, charcoal grey and pink. The HKK style sweatshirts were sold in black, navy, light blue, light grey, pink and red. The JMK sweatshirts are reversible microfiber with polar fleece lining and a removable hood with elastic or retractable drawstrings. Style JMK was sold in black only. The sweatshirts have sewn-in neck tag that reads “Gogo Sports” and city names printed across the front of the garment.

Sold at: Clothing retail stores in California, Colorado, Hawaii and New Mexico from January 2008 through December 2009 for between $14 and $27.

Simplicity Cribs Recalled by Retailers; Mattress-Support Collapse Can Cause Suffocation and Strangulation- One Infant Death Reported

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission (CPSC) is announcing the recall of all Simplicity full-size cribs with tubular metal mattress-support frames. This recall includes fixed-side and drop-side cribs. These cribs pose a risk of serious injury or death due to entrapment, strangulation, suffocation and fall hazards to infants and toddlers.

The crib's tubular metal mattress-support frame can bend or detach and cause part of the mattress to collapse, creating a space into which an infant or toddler can roll and become wedged, entrapped or fall out of the crib.

CPSC has received a report of a one-year-old child from North Attleboro, Mass. who suffocated when he became entrapped between the crib mattress and the crib frame in April 2008. CPSC is aware of 13 additional incidents involving the recalled cribs collapsing due to the metal mattress- support frame bending or detaching, including one child entrapment that did not result in injury, and one child who suffered minor cuts to his head when his mattress collapsed and he fell out of the crib.

CPSC staff urges parents and caregivers to stop using these cribs immediately and find an alternative, safe sleeping environment for their baby. Do not attempt to fix these cribs.

Due to the fact that Simplicity and its successor, SFCA Inc., are no longer in business, CPSC has limited information about the number of cribs sold. All Simplicity drop-side cribs have previously been recalled for a hazard involving the drop side. Simplicity drop-side cribs could still be in use by parents or caregivers who are unaware of the recalls or by those who received a repair kit to immobilize the drop side from Simplicity when the firm was still in business. This recall involves ALL Simplicity cribs with tubular metal mattress-support frames regardless of model number.

Some model numbers are followed by letters, indicating the color or finish of the crib. The name "Simplicity Inc." or "Simplicity for Children" appears on a label on the crib's mattress-support frame and/or the crib's end panels. The cribs were manufactured in China.

The recalled cribs were sold at Walmart, Target, Babies R Us and other stores nationwide for between $150 and $300. Consumers should contact the store where the crib was purchased to receive a refund, replacement crib or store credit.

Important Message from CPSC:

CPSC would like to remind parents not to use any crib with missing, broken or loose parts. Make sure to tighten hardware from time to time to keep the crib sturdy. When using a drop-side crib, parents should check to make sure the drop side or any other moving part operates smoothly. Always check all sides and corners of the crib for disengagement. Any disengagement can create a gap, which could fatally entrap a child. In addition, do not try to repair any side of the crib with tape, wire, rope or by other means. Infants and toddlers have died in cribs with makeshift repairs.

Graco®-Branded Drop Side Cribs Made by LaJobi Recalled Due to Entrapment and Suffocation Hazards

Name of Product: Graco®-branded drop side cribs made by LaJobi

Units: About 217,000

Manufacturer: LaJobi Inc., of Cranbury, N.J.

Hazard: The drop side hardware can break or fail, allowing the drop side to detach from the crib. When the drop side detaches, a hazardous gap is created between the drop side and the crib mattress in which infants and toddlers can become wedged or entrapped, posing a risk of suffocation and strangulation. In addition, children can fall from the cribs when the drop side detaches or fails to lock.

Incidents/Injuries: CPSC and LaJobi have received a total of 99 reports of drop side incidents, including hardware breakage and drop side detachment. There were two incidents in which children became entrapped in the gap created by the detached drop side. Both children were freed by their caregivers. There were six reports of children falling due to drop side failure, including one report of a mild concussion.

Description: This recall involves LaJobi-manufactured Graco® wood cribs. The full size cribs were sold in cherry, espresso, natural and white finishes. The production date, item number, purchase order number and finish name is printed on a label affixed to the footboard or headboard. "LaJobi" and the crib model name are printed on a product sticker located on the stabilizer bar or bottom rail of the crib. Affected models are listed below.

Sold at: Children's product stores and other retailers nationwide from February 2007 to March 2010 for between $140 and $200.

Manufactured in: China and Vietnam

Remedy: Consumers should immediately stop using the recalled cribs and contact LaJobi to receive a free hardware retrofit kit that will immobilize the drop side. CPSC urges parents and caregivers to find an alternative, safe sleeping environment for their baby.

Gap Recalls Baby Swimsuits Due to Strangulation Hazard

Name of Product: Baby swimsuits

Units: About 6,500 in the U.S. and about 480 in Canada

Retailer: Gap Inc., of San Francisco, Calif.

Hazard: The swimsuits have halter straps that were manufactured too short causing the plastic ring located at the neck of the swimsuit to press against the child’s throat and obstruct the airway. This poses a strangulation hazard to the child.

Incidents/Injuries: The firm has received two consumer complaints. No injuries have been reported.

Description: The baby swimsuits were sold in two styles: number 706260 is blue and white and number 700452 is red and white. The style number can be found on the label located on the swimsuit. Both swimsuits were sold in infant sizes up to 24 months and are made of a synthetic knit stretch fabric of polyester and spandex. The straps are made of the same material as the body.

Sold at: The babyGap, GapKids, Gap, Gap Outlet stores nationwide and online at www.gap.com from February 2010 through April 2010 for between $17 and $20.

McNeil Consumer Healthcare and FDA notified healthcare professionals of a voluntary recall of certain over-the-counter (OTC) Children’s and Infants’ liquid products manufactured in the United States, including Tylenol, Motrin, Zyrtec, and Benadryl products. Some of these products may not meet required quality standards. This recall is not being undertaken on the basis of adverse medical events.

However, as a precautionary measure, parents and caregivers should not administer these products to their children. These products were distributed in the United States, Canada, Dominican Republic, Dubai (UAE), Fiji, Guam, Guatemala, Jamaica, Puerto Rico, Panama, Trinidad & Tobago, and Kuwait.
